Please Note: REQUIRED $300.00 DEPOSIT applied to rental costs. The customer agrees and understands that efforts to collect any and all outstanding debts and invoices is without any limits of dates and time; there are no limits of time in the collection of outstanding debts of customer.

Please Note: All prices are in U.S. Dollars. Applicable U.S., State, and domestic and foreign taxes, including Value Added Taxes in certain countries, are not included, are additional; and will be added. Any and all Rates are subject to change without notice.

Please Note: A $150.00 cancellation fee will be charged immediately if a phone order is cancelled 3 days prior to the designated "Phone Delivery Date" as stated on the original application; and I hereby pre-authorize my credited card company that I approve of this cancellation fee in advance.

Statement of Authorization
I hereby authorize PlanetFone and Planet Cell Phone Rentals to bill all cellular phone rentals fees; shipping, receiving, late penalty fees, taxes, any and all residual calls and data charges, and any other applicable charges and fees to my current credit card as directed above. I understand that any charges billed to my designated credit card will be subject to other charges that appear on the account in accordance with the terms and conditions governing the account. If my credit card is denied, expired, and/or does not process payments; I am still responsible for all payments in full and must immediately provide another credit card or payment in full. Each time I rent a cellular phone, it will be subject to a credit card authorization before my reservation is confirmed and the cellular phone kit is delivered. If I cancel my order, I agree to pay any incurred costs and cancellation fees in the amount of $150.00. If my card status changes, or if my card is lost or stolen, I agree to notify Planet Cell Phone Rentals and/orPlanet Phone and the appropriate credit card company immediately. If Planet Phone and/or Planet Cell Phone Rentals changes ownership, modifies it's name, or merges with another company I agree to pay any and all outstanding invoices presented by the appropriate companies involved. If I elect to terminate this credit card authorization to bill my account, I will notify Planet Cell Phone Rentals, PlanetFone/PlanetCellRentals and the appropriate credit card Company. However, this does not resolve me or my debts which have encurred. The customer agrees and understands that efforts to collect any and all outstanding debts and invoices is without any limits of dates and time; there are no limits of time in the collection of outstanding debts of customer. The customer agrees that he/she will not: a. Effect any repairs or modifications to the cellular phone or other rental equipment; remove or tamper with the SIM card and/or any sim card certification markers. If SIM card is removed, damaged; missing and/or tampered with in ANY MANNER, customer will be in violation of the rental agreement and will be charged a minimum of $300.00 for sim card replacement; along with any other repair cost associated.
